Help Contacts' #s showing up as "NAME <+NUMBER>"

First, this is only for SOME of the contacts that I imported from my old phone. None of the contacts I've entered since getting the phone have this problem. And it started happening a while after I had imported the numbers.

This only causes problems from the texting screen b/c if I try to call or reply to a text from that screen, it dials/sends a text to the numbers that correspond to the letters of the name, instead of the actual number.

For example, if I had a Jack in my phone, he would show up as """Jack <555-5555>" and if called/texted him from the text screen, it would dial 5225 ie JACK.

When I try to view the contact info, it asks me if I want to add the contact. If I do, and then try to change the phone number to get rid of the "NAME" in the number field, it creates a new entry instead of editing the one I just created. So if I tried to do the above to Jack, I'd get 2 entries: Jack w/ the correct info, and """Jack <555-5555>.

I've tried deleting Jack from my phone and double check that's he not there by dialing the number and it doesn't come up. I put Jack's number back in my contacts and DAMMIT! """Jack <555-5555>." is back in my phone.

Even if I call/text the 2nd entry of Jack w/ the correct info, it groups them under the "Jack" entry, so that I can't reply to texts from Jack b/c it sends the reply to "Jack" whose number is 5225.

So the list of problems:
1- """Jack <555-5555> is somehow stored in my phone so anytime I add 555-5555 as a contact it defaults to that entry first, then let's me create a 2nd correct entry.

2- I can't find """Jack <555-5555> in my contacts. If I delete the contact, it's still there somewhere, just no in my SIM card, Phone Book, or Gmail account.

3- I can't edit "Jack" contact number b/c it creates a new entry if I change the number.

Any ideas?
 
Are those contacts on your SIM card? I had that problem of it creating two entries. I just finally deleted everything off my sim card and I don't have that problem. Everything is saved on Google anyway.

No. I erased the SIM contacts a while ago and the problem is still there.

A new bit of info: I cleared out the data in contact storage and those bad contact entries are still there. Everyone else shows up as just the number. The bad ones show up as """Name <+number>"

Can anyone shed light on where my phone is getting that information from? It's not from my SIM card, not from my SD card, and s not from Google contacts.
