I did a factory reset and re-installed Google Maps, Gmail and Chrome, all of which worked fine; next I installed Evernote. When I opened Evernote I got this message, Evernote requires Memory card: Please insert a memory card before using Evernote.

When I remove my SD card I get notified: removed SD card
When I put it back in I get: SD card safe to remove
When I go to Settings -> Storage, and hit Mount SD card, I get: SD card safe to remove and the Mount SD card button is still pressable.
When I take my SD card out in the Storage menu, the Mount SD card button becomes unpressable and I get the SD card safe to remove message.

What that tells me is that the phone is recognizing that the SD card is there but can't mount it.

Are there any files on it now? I'd recommend backing up anything that is on the card to a PC and then reformatting it in the phone. If that works, then you can move your files back to the card. :thumbsupdroid:

It might also be an indication that the card is about to fail or already has. :(

Also, just wondering: why was it unmountable after the factory reset?

Could have been anything ... corrupted file system, bad symbolic link, flaky SD card, evil spirits ... I'd keep an eye on it, though. SD cards have a habit of dying suddenly without much warning. If it shows any unstable behavior like write or read errors, or failure to mount again, then think about replacing it before you lose important information.
