Help Gnex Update 4.0.4. Question. Freezing.

Phone Info
Galaxy Nexus
Version: reads 4.0.4.
Baseband: I9250UGLC1
Kernel: 3.0.8 g034fec9 android-build@vpbs1 #1 SMP PREEMPT tue mar 13 2012
Build: Android Revolution HD 3.0.0 GSM by mike1986

Since I've gotten my phone, it is always pushing the android 4.0.4 system update. This seems paradoxical because the version already reads 4.0.4.

When I attempt to do the install and restart, the phone begins installation and then shows a picture of the green android character horizontally, with an exclamation (!) point within a red triangle above the android character. It freezes at this screen, is unresponsive to touch, and requires taking the battery out to restart.

Can someone help me get the update properly installed? Do I need to root my phone?

I'm unfamiliar with rooting or android software in general, so please me gracious and let me know what more information I can provide! :D

You should try to boot into recovery by holding the power and volume up and down buttons.

Doing so you should try and apply the update zip, which should be on your phone.
 
Easiest solution is:
1) backup your data [saved at /sdcard]
2) download & flash the images from https://developers.google.com/android/nexus/images
3) restore your data, root [if wished], & enjoy.

I'd do latest 4.3 images bc if you do the 4.0.4 images you'll get follow-up OTA update(s)

PS the exclamation android is a common phenomena; something changed in /system [or perhaps the recovery itself]. Newer OTAs have signature checking which can cause such fail(s).

Clockworkmod Recovery v5.5.0.2

--Installing: /sdcard/update.zip

Could you provide a link for instructions on flashing? I've read a few guides so far. Do the instructions at the following link look sufficient?

according to your baseband from the first post, you have a maguro(yakju) galaxy nexus, and you are already rooted... easiest thing to do is to just flash an updated rom...
