Help Lock Screen Issues

I am having major issues with my lock screen. When it used to work properly my lock screen had the time on it and if it was plugged in would tell me the battery charge as well...lately it no longer does that...it just has a black screen that if i swipe left is camera and right to unlock and if I hold the button down an arch of a few choices show up. I have tried turning the phone off then back on and sometimes it fixes it temporarily but then goes back to the black screen when I try to unlock it. It also makes a noise like something was running and stopped at the same time as the screen changes...how do I get my phone back to showing me the time and battery charge etc and not this pathetic black screen of death...UGH!!!!

Did you used to have any lock screen widgets enabled, or was it just the stock digital clock on your lockscreen?

What OS version are you on? 4.1, 4.2, 4.3?

I'm a little confused by the fact that you say you swipe left for camera and right to unlock....and that there are any options on holding the lock. None of that is true for my default lockscreen. On mine, if I swipe the "lock" any direction it unlocks. If I swipe the screen right I get the camera, if I swipe the screen left I get additional lockscreens or the option to add lockscreen widgets.
