Help SD Card unexpectedly removed

On my LG Optimus L9,, about every ten minutes the SD card is unexpectedly removed. I have formatted the card multiple times, done factory resets of the phone, and bought a new sd card,which worked for a few days, then it started haopening again. Got any suggestions?

I have the identical problem with my Motorola Moto G 2nd gen....

Hopefully The warranty is still valid and I can turn it in.
