With those 4 pages of permissions for the gallery alone, I would have to say you've definitely been compromised! (Anytime I see 'run at startup' attached to something it normally isn't, I get suspicious). Sounds like you've got a major nasty onboard, beyond the GF. If a factory reset didn't clean it up, looks like you're going to need to do a total wipe and load a fresh, known clean rom for your phone.
All those overseas file listings are highly suspicious too!
Bottom line, I wouldn't use that phone for anything until its cleaned up. You need to consider changing every password associated with this device as well. I'd do that from a pc/laptop that I know isn't infected and the gf has no access to. Skeery sheit bud!!!

Ooh, new avatar! :) Sorry, that was off-topic.

The easiest way to hack a phone is via your Google account, so I would take steps to protect that first: use a computer rather than the phone, log into the account, de-authorise any devices you don't recognise, enable two-factor authentication and change the password. Once you've secured the account you can worry about securing the phone.

One word of warning: changing the Google password and then resetting the phone needs to be done with care, as that can look suspicious to Google and result in you being locked out of the phone for a few days (I think the assumption is that if a phone is stolen the real user will change the account password and the thief might reset the phone, so the two happening close together in time is treated as suspicious). I'd suggest removing the account from the phone before resetting, which is the normal way to avoid factory reset protection issues.
