A contact in my phone disappeared? ??WEIRD

I just had something weird happen. My wife called me. And her name did not appear, just the number. I went back and looked, and she was no longer in. my contact list. Any ideas how or why this happened. Motorola Droid

This might be far fetched but this is the only things I can think of:

1. You mistakenly erased her.
2. bad sector on where the contact is stored.
3. She called from another phone.

Sorry that's the only things I can come up with.
