after a simple app for commercial use (feedback)

Hi, Could anyone please tell me if there are apps at there that can have a text box so people can write there review in the tablet and have name,number ect. Just have a hotel and wanting to get feedback so I have a samsung tablet at the front and if they want to they can put a message in for a review.

practically any memopad app like Google Keep can be used. Just put a widget.

Yes, to meet the requirement as you just said, any memo app is enough. For example, the Action Memo widget, which is a stock widget on Samsung device, is enough.
Certainly if you want an app which is more user friendly to your customer, you may ask someone to develop another simple app just for you.
