Not sure.
But can you pull down the notification bar and touch on the icon? If you can see what pops up, you can also long press on a notification to gain access to additional options to control whether or not to allow notifications.
If you cannot gain access to the notification, with the notification active go into the system settings and find the Memory section. Pick this option and see if you can find the mystery app listed in there.
