App/SD frustration

I have a galaxy S4 and i love the phone. I think its a fantastic piece of tech and its been rly useful, but thats not why i'm here. Recently my phone randomly decided two installed apps didn't exist, Despite the fact that when i checked the settings the data was still there. It had an image of a folder with the android logo on it with an sd card next to it where the app's logo should have been. Believe it to be annoying but not something i'd miss, i deleted the two apps data. However i recently restarted my phone after the phone froze for a bit. After it started up it had happened again only this time to a good 16 apps i believe. none of which work. i spent an hour tracking down the app, deleting the old data then re installing the apps, But i need to know what the hell is going on and how to stop it.
So my question is, what the hell is going on and how do prevent it from happening again?

From your post I am unclear if you are using an external SD card but assuming this is the case. Hopefully these suggestions will help.

1. Are you using any apps2sd apps or link2sd? If so you might try removing and reinstalling that particular app.

2. Have your tried replacing your external SD card? This does sound like the card may be going out and if so replacing the external card might save a lot of headache down the road.

Was your phone connected via USB cable to a computer when this happened? If so, that could be the problem. If it happens again, try unplugging the phone from the computer and see if the problem goes away. (I don't have the same device you do, but my Motorola Atrix 2 does this, while its predecessor, my Atrix 4G, does not.)
