Blue "Block Caller" screen

Hey all! New to the forum, long time galaxy owner. This morning I got a new screen on my phone. I haven't downloaded any new apps or done any updates recently... to apps or OS. I made a phone call, locked my screen, and when I unlocked it 3 mins (or so) later, the screen was there. I can't make it go away and can't trace it back to any app in particular. I tried to take a screenshot of it to show the local Verizon store reps and I got an error message stating that I wasn't able to take a screenshot due to security permissions. The agent looked at my phone and put it in "Safe Mode". No blue screen came up when I made a call. She proceeded to tell me that safe mode disables all third party apps therefore the blue screen was being controlled by an app that I downloaded. Anyone ever seen this screen or just so happen to know where it may have come from? It doesn't do anything but annoy me because I don't want it there.
