Help Bluetooth Aggravation in Note 5 - help please!

My phone is attempting to connect by bluetooth with a phantom device (my old car) every 20-30 seconds. It rings a tone and shows me a notification that it cannot connect. I cannot find the pairing in the list of devices and I cannot make it stop! If I shut off bluetooth, the behavior stops. Only when I have turned it on that this is occurring, so I know it's a bluetooth problem. I have cleared the cache and reset the device several times. Even though this is attempting to connect to my old car, it has been a long time since I had it connected, but this is a recent phenomenon.
 
Scratch that. Misread that you couldn't find the list, not that device wasn't there. Do you have any 3rd party apps managing Bluetooth?
 
Something to consider trying:

Settings - > connections - > more connection settings

At the bottom, "reset network settings".
Says it will clear all your wifi, mobile data, and Bluetooth settings
