Calls not coming in?

I recently got this phone and am experiencing issues with my calls. When my girlfriend tries to call me it takes 6 or 7 tries for my phone to even ring on my end. She says it rings twice and then goes to voicemail..

I checked my settings and she's not in any block list or auto reject list.

What could be the issue here?

Who's your carrier?

The last two times we upgraded the wife's phone, we had to call AT&T to reprovision the SIM because the rep at the store hadn't done it correctly.
