disappearing home screen icons and folders

Hi all,

This is crazy making. Sometimes when I restart my phone, all but a few random icons on my home screens are gone. It is happened about 5 times now.

I spend lots of time organizing my apps on my home screens so after a restart and my screens are completely empty, save a few, it is devastating.

I have a Galaxy S6 5.1.1

Any idea what is going on and how to stop it?
