It's the Best Buy branded version of the Seidio case, the Platinum Series case. It's made by Seidio and even though it doesn't have their logo it's got the same soft touch plastic and quality. Here's a link to case on Best Buy LINK .
I don't have the original packaging but it's in 100% condition, I only had it on the phone for a few days. No chips or scrapes, I just put some felt furniture pads on the inside of the case to make it fit more snug and protect the phone. It's a great case and great quality, I just prefer the phone without it.
