Folders for SD files

You need a MicroSD card.
I've always used HTC stock file manager which does everything I need so have never used other file managers so can't comment on X-plore. If you're having problems with it try other file managers or, if you have a PC/Laptop, an alternative option would be to either connect your device to it, or unmount your SD card, remove it from your device and use an adapter to access it from the PC/Laptop, to create your folder/s and copy/move files.

OK, I got a couple of folders set up with the PC and I managed to add some images. Does this mean I have to do it that way every time? I would like to be able to do this on the phone rather than the PC at least part of the time. By the way, I have pretty much given up on actually plugging the phone into the PC. It's just too much trouble to deal with those crazy cables that don't fit. Even the cable I am using can be hard to connect. Instead, I have resorted to using the SD holder to insert the card into that slot the PC has, which presumably means that I couldn't copy anything from the phone's internal storage to the PC. It would be nice if the manufacturers would add some kind of tab or lever to make it easier to open the phone.
Have you tried any of the other file manager apps from the Playstore?
Does your whatever it is phone or tablet not have a built-in file manager that can create folders, move files about, etc.? Most Android devices do include some sort of file and folder management app.
WHOA: when you asked "How do I keep it from disappearing?" alarms went off in my head.
If you are taking photos and they are disappearing from your card, and they are not in the DCIM folder with the other photos you've taken with the camera, it usually means that you are exceeding the storage limit of the card. As it writes the new files, old ones are written over... never to be seen again.
This is common with counterfeit microSD cards: dishonest sellers will reformat a 4 or 8 GB card to read as if it's 128 GB or more. Your device may even show that there is plenty of room available. But you cannot put 15 liters of water in a 5 liter bucket, even if the bucket is labeled "20 liters" - once the physical limit of the card is reached, you will experience data loss. And fake cards are everywhere! Right now the highest capacity microSD card available is 400 GB from SanDisk, and it's about $250 USD. But if you look on eBay, there are plenty of sellers selling 500GB and even 1TB microSD cards for $50! That is data loss waiting to happen.
You can install a free app called SD Insight to determine if the card is legitimate or fake. If it is legitimate and you are still losing data, it's possible that it's just dying. Flash memory does have a life cycle, measured in read/write cycles. Exceed that and the risk of data loss increases.

Actually X-Plore can create new folders just fine. Navigate to the folder you wish to put the new folder in, the scroll through the tools bar down the side until you find the "new folder" button. Or just long press on the folder you want to add the new one to and the pop-up menu will have "new folder" as one of the options.

I also had a genuine Samsung card that I bought with the tablet that died suddenly and permanently after 3 months of very light use.
