You should be able to find the username by looking in the accounts & sync settings of your phone. Select the "google" account and you will see the username. To reset the password you'll have to contact Google. Try to log in to GMail from a browser select the "can't access your account" link. You'll need to change the password on the phone afterwards.
If you can't get the name from the phone, try the same thing only tell them you can't remember your username. If you set up a recovery email address when you created the account then you should be able find out the username this way.
