I purchased the SIM for Monthly 4G from T-Mo website and activated the $30 from T-Mo website. No need to visit Walmart.

I just did it all at Walmart. Bought the phone there, took it home and activated it on line. Total time was about 30 minutes, if that much.

You can use CC or debit at both the T-Mobile online website and at their physical walk-in stores.

You cannot get this special web only deal though at a physical T-Mobile store.
