Help How do I remove stubborn app ONLY on desktop? V4.3

How do I remove a stubborn app (the old Amazon app) that is ONLY on the desktop/list of all apps on device (and NOT listed under Settings > More... > Application Manager)? It does NOT delete when you long-press and drag it to the trash can either. I also tried installing a third-party app uninstaller, but it couldn't find it listed as an application. Nor could I find it under Device Administrators, just in case that needed to be removed.

Phone is Samsung Galaxy Note 2 (SGH-T889) running Android Version 4.3.

The blue circle is the old app and the green square is what replaced it...

I don't think it was installed within the Amazon Store app, as I don't see that anywhere on the phone.
 
Not listed under settings app manager...that's weird.

On my phone there are tabs in application manager for "downloaded apps", "running", "turned off" and "all apps"....
Did you look in "all apps"? How about in "turned off"?

Was NOT in Turned Off, but I failed to check "All" previously - found it, turned it off, stopped notifications and it moved it to Turned Off and took away the icon from the desktop. Thank you!!!
