iPhone 5s and ETF

If I were to sign a new contract and get the iPhone 5s, if I wanted to end my contract after the 14 trial period, what would my ETF be? $350?
Do I have to return my phone? Or pay anything else?

Yes $350, then minus $10 for every month on contract. You don't have to return the phone, its yours now. You'll still be billed for your 14 days with cell service. If you quit within the 14 days, then you'd have to return the phone.

You would have to pay the initial cost of the iPhone 5s with a 2 year contract:
16 GB - $199
32 GB - $299
64 GB - $399

Plus $350 for canceling if you cancel on the 15th day, and for the cell service you used.
