LG stylo 2 volume, headset etc issues

Bought it October 2016. Few months ago every time I plug in any headphone, the volume constantly goes up and down  on its own, or the music apps stop playing or reset. 
Now the plug charge is very loose so I wonder if the whole bit inside is loose, jack included. When I remove headphones, I'll click something anything on a screen and usually its play music that will suddenly start playing.
I have the insurance so I may send phone in for replacement unless I get help here

I would take it to your carrier and get it fixed or replace. Thats best since you have the insurance and lg covers it 1 year from your purchase date under warranty.

You could try looking inside the jack to see if there is some lint. That could cause the pin to not seat right.
