Medical Apps/Notes

I am getting ready to go to the doctors for a physical and have lots of things I want to bring up - are there any apps that are good for keeping notes for this?

Or maybe just a general note taking app that would be useful?

Colornote is great for general notetaking. But if you have a specific condition, there are apps to log stuff like migraines, or bipolar/EIP mood swings, or asthma, etc. Like My Headache Log, where you enter info everytime you have headache or migraine, and then you get really neat charts showing your most common symtoms, triggers etc.
