How do I stop duplicate icons on home screen. It runs under Qualcomm

Long press on the icon and move to X or trash that appears onscreen

Dannydet: I have same problem duplicating widgets. When I press widget and drag to remove or x, it duplicates. When I open a widget on the bottom row it duplicates.
