My phone keeps turning off

I have a Samsung Galaxy W and it keeps turning off and saying 0%, even though it has 100%. Whenever I use it for about 5 minutes or less, it turns off. But when it's on charge and I use it, it doesn't happen at all! What is wrong with it? Is it the battery? Btw the battery does look a little 'fatter' than it would look when it's new.

Everything was pointing to a possible faulty charging cable and/or charger/USB port until your mention of the "fatter" looking battery. That's a sign of a defective piece, which is in need of replacement.

Moisture likely caused the issue, whether from mere humidity being used outdoors, or sweat or other water working its way into the battery compartment.
