Need Guidence on Android App

I am looking for a free android app for a charity project that will gather the data from various locations in Africa and will submit it to a one centralized location, form where I can download the final compiled excel sheet with all submissions.

In simpler words it will be google forms but for android.

Data I need is

[GPS Coordinates]
[Time Stamp]
[Date Stamp]
[Text Input]

Hi,

Hmm, there might be some apps out there that gather your current location data but not sure if you will be able to find one that can do for any location... I guess it could be good idea for a new app.

There is something called ODK - open data kit, that I found digging internet. It appears promising, but for that, I'll be hosting the server for receiving filled forms on my own. (for which I have no issue).

But there is not a single definitive guide, that shows a layman on how to implement it/deploy it.
