Root Need to get back stock ROM

I need some assistance. I have rooted by RAZR MAXX HD, installed Voodoo, and selected temp unroot to try to update with the OTA JB. I get the dead Droid with the red triangle.

I ended up doing a complete restore (Settings > Privacy > Factory data reset), but that clearly is not doing a complete fresh restore as the OTA update still gives me the dead Droid. How do I get my phone back to original state so I can start over fresh?
 
Ok, didn't realize that there was an update for LinuxMacDroidRazrHD.sh. I have updated the script and I am now able to do Fastboot restore to 0.6.25 (ICS 4.04 Official). Will let you know if I am able to get to JB.
 
For those that may be interested, updating LinuxMacDroidRazrHD.sh made all the difference. I was able to restore the stock ROM, re-root, and update with no dead Droid.

One tip, when rooting with LinuxMacDroidRazrHD.sh, it will hang until you hit Control-C on the Mac and that will prompt the script to process completely.
