Note 4 update

I just clicked on settings to see if there are any updates for my note 4 and this screen appeared:

Can anybody suggest what this means?

YEPPERS:
Something other than OEM Firmware/Software has been installed,or,the phone has been rooted at some point.
If you're reasonably sure this is not the case,then I do not have an explanation.
If you bought this phone new from your service provider/retailer,return it for a replacement & insist on a new,not refurbished device.
 
Oh, i did root it a while ago. Is it possible to unroot it?

Ive just done that but it has made no difference. Supersu said it would delete itself after unrooting but it hasnt and when i go to open it it crashes now.

Yeah,that's not gonna go away:
Ever since Samsung started installing the KNOX software on their phones (& possibly before that),there's no way (typically) to change back to a 100% out-of-box condition.
You can flash whatever firmware,OS,updates,ETC.... by side-loading,but,not through typical OTA's.
Rooting/ROMMING is detected by KNOX,& AFAIK,there's no way of resetting the tattle-tale circuit breaker,if you will,that indicates that the device has been rooted/ROMMED/altered from the factory OS.

This virtual fuse lets Samsung know that the device has been altered & in most instances,voids any warranties & ceases factory OTA's/updates.
From here on out,you'll have to side-load updates.

I'm not sure if you can update via KIES with a tampered device.
As for OTA updates,I'm 99.99% certain you won't be seeing those any more.

Neither of the above scenarios are the absolute end to getting updates,check out the root section here @ Android Forums or over @ XDA Developers.

It's been a little while since I've owned the NOTE4,but,I believe you'll be able to install/side-load updates via ODIN:

Just to confirm, I recently used this process to revert my phone back to stock KitKat and it worked flawlessly. I can also say that the OTA updates were working afterwards (much to my annoyance) ;)
