Help "OK Google" doesn't work after placing/receiving phone call

I am trying to help a paralyzed family member use Galaxy with strictly voice commands, as they have no use of their hands.

"OK Google" is working great to read/send texts, answer questions and place calls. BUT ... after a phone call is made or answered, the phone doesn't recognize "OK Google". If I go into the Google app settings, "OK Google detection" and turn "From any screen" off/on, "OK Google" is recognized again.

Note: I also have
  • "Personal Results" turned on in "OK Google detection".
  • "Incoming Call" under "Hands Free mode" turned on in Android settings
  • "Messages" under "Hands Free mode" turned on
 
There have been a number of problems with "OK Google" reported recently although different to yours in that they were a case of "OK Google" responding even when switched off. They were resolved by 'uninstalling updates' to the Google app, reverting to the 'Factory version' followed by updating again so it may be worthwhile you try that as a first step.
