Your battery needs recalibration, it's stuck with a corrupted battery stats file or the voltage sensor can't read the voltage correctly.
If you have a charging cable with status LED, it should let you know when the battery stops taking charge. The charging status LED on the phone is usually useless in this case.

How do I recalibrate it? I thought I had the issue fixed. Now I'm having trouble with it charging/not charging. I plugged it in before bed last night (7 hours) and all night it would charge then stop, charge then stop. It only made it up to 50ish%. Now it's down to 28% and doing the same thing.
