Phone gets uncomfortably warm during phone calls!

I noticed that it happens more so in the day time, maybe its the hot weather here, but then again I don't get too many phone calls at night time, so I cant say for sure. But it gets really warm, approaching hot, when talking for extended periods, then holding it next to cheek becomes really uncomfortable. Has anyone else noticed it? Its a powerful phone, so dont know if anything can be done about it or not.

BTW, does anyone know a good app to tell phone temperatures and measure current in/out flow in amps!!

There is an app called Androsensor that gives you temperature and other things also. I haven't had it long but seems to do alright.
