Help Phone locks itself while in use

(Moto X 2nd gen, Lollipop)
I've looked through google and this forum for phones locking themselves, and while I've found a few threads on it none of them have shed light on mine. I set the lock timer for 30 minutes, but sometimes it locks itself in as little as three. It doesn't even have to be inactive for it to lock, sometimes I'll be in the middle of typing something and the screen will lock. I tried turning off the power-button-to-lock setting in case maybe there's a grain of sand or the like stuck under the power button that was triggering it, but it still happens. I saw in another thread that it was someone's in-pocket-direction setting, but as far as I know this phone doesn't have that. Also, it happens both on my pillow as well as in my hands, so I'm pretty sure it's not a nearby magnet causing it.
This isn't a huge problem in and of itself, it's quick enough to reenter my PIN, but above its mild annoyance I wonder if there's a larger underlying issue.

Perhaps you're covering the proximity sensor ("just above the top of the screen")? See page 17 of the Users Guide.

Yep, it does, as Thom indicated. It could be the prox sensor? Do you have a screen protector on the phone that might be covering it? (I believe it is to the left of the earpiece at the top of the phone.) Or a case that might be covering it?

That said, it could be a bad phone or a bad sensor, so don't rule that possibility out.
 
I could not find a picture that shows the exact location of the sensor on this phone.

I use Tasker and have a Tasker task that will enable or disable the sensor. This then allows me to keep the screen lit when making a call and otherwise turn the screen off when the senor is triggered.

I don't have a screen protector or case (yet), and I didn't see anything blocking it but I wiped the area down with some fleece, and it still happens. Sometimes it will even try to lock itself when it's already locked, or at least it makes the little locking click noise.
Is there a way to change which direction is considered in-pocket, doogald?

Edit: I noticed too that before it goes black and locks, I'll see my home screen wallpaper, and it looks like it's trying to resize or adjust itself. It will stretch vertically a bit and go back.
