Help Phone Screen is stuck on Easy Mode

My Samsung Galaxy S5 is having a problem where when I make a call it uses the Easy Mode screen showing only the options for Speaker, End Call, and Bluetooth. I do not have the options for Add Call, Keypad, or Mute. I have attached a screenshot showing the issue, the red party is me blurring out the name and number of the caller.

This is now the third time this has happened to my phone. The previous two times I was forced to do a factory reset. A regular reset does not fix the problem.

I have had this phone for just over a month but it seems like this issue starts about every two weeks or so after I do a factory reset. I have not changed any settings in my phone since the last time I made a phone call and I do not see any new App updates.

have you tried to reboot??

i received by brand new s5 on easy mode. . i rebooted after.. no issues since then. :)
 
Yes, I've rebooted the phone and I've also done the Reset settings option but that did not fix the problem. The only thing that seems to work is a factory reset but given the recurring nature of this problem I'm trying to avoid doing this yet again.
 
Yes, you gave me the hint I needed to solve the problem. I went into my Settings then Applications and Default applications. TouchWiz home was selected which is correct but when I hit Clear and changed Home from TouchWiz home to TouchWiz easy home and then repeated the steps to change it back to TouchWiz home it worked.
