Had you been trying to purchase anything?

It looks like you were using a browser at the time. I've not heard of an ad that attempts to buy something, but if this only happens when using the browser I'd clear your browser's cache for certain and see whether that is sufficient. I'd also ask whether you have installed or updated any apps shortly before this started? Especially any that have the permissions to make in-app purchases.

No, I've not seen this before, so just thinking about how you might work out what is responsible.

Looks like a phishing ad to me, hoping that you'll click on whatever 'retry' option they present - hand over your card details and then they can have a spending spree on you.

When it's happened, was there anything else, like something prompting you to do some action, or just the gray box saying 'purchase has failed'? Does the gray box go away on its own?

No, I've never seen it. But I don't think it rises to the level of having been hacked, or anything like that.

Were you using the same app each time it happened? Which app(s)?

Did it start soon after installing something new?

Whatever you do, don't click anything unexpected--like that gray box or anything it spawns!
