Please help - Voice Mail

I have the the visual voicemail icon constantly popping up on my phone and the voicemail is empty. How can I stop it? Any suggestions please?

It's a Motorola Droid Ultra version 4.4.4. the carrier is Verizon.

I have a Motorola on TMO. It does the same. If I turn it off, change anything, there will be a new voicemail notice. It's the phone, not a caller. Moto needs to fix that.

I had Voicemail turned off for a long time and I got those messages. That's how I know it's the phone.
