Help Re appearing bubble

this pic keeps re appearing in my photos app on my galaxy S4. I delete it, and when the phone re starts, its back again. What is it and how do I permanently remove or delete it.

This is also popping up on my wife's phone, any ideas anyone? She has a bunch of apps that my son uses on her phone that could be creating it but I don't really know. The folder that it creates for itself is "ndownloads" if that matters.

"ndownloads" does matter. I pluged my phone into my computer. My phone has an sd card in it so two bars appeared, one said card and the other one said phone. I clicked on phone to open it. I scrolled down to the file labeled "ndownloads" and opened it. There it was, that emoji as shown in my post. I deleted it and so far it hasn't returned to my phone or picture folder.
