Receiving call disables silent/vibrate mode?

I have tried multiple apps and am now noticing the same tendency for all of them (silent mode, mute, silent toggle widget): I'll have the phone in silent or vibrate mode it doesn't matter which one; and if I receive a call it take that function off and I am stuck with the volume turned up.

So basically if i'm in a meeting and have it in silent mode and receive a call, ignore it, then receive another call my phone will ring.

Is there a specific setting I'm missing?

I had the same issue when using AudioManager app.
