I would honestly need more information to help. When your phone was repaired, did the technician format your phone? If so, and assuming you downloaded the app from the Play Store, Google should automatically restore the app to your phone.
If you are asking to get back the individual notes after a restore, I'm afraid that isn't possible. A format wipes all personal data to allow for a clean install. Sorry!
