rsd lite cant boot to flash mode

Trying to flash back to stock using rsdlite. I keeping getting error that the phone can't boot to flash mode. But the phone is in ap flash mode. Rsd reads the phone. Then I go to flash, it boots to flash mode, and fails. Rsd doesbt realize the phone is ready

Any one have this problem before?

What version of RSD are you using and which fxz are you trying to flash.
