Without some more specific information such as:

• the model of your phone
• the version of android you are running

It is difficult to help you.

However if it is the same as my S3 you need to:

• Go to your apps page
• Press widgets at the top
• Go to the first few pages where you should find 'Assistive Light'
• Press and hold the icon for a few seconds and then drag it to an appropriate place on your home screen
