Help "Screen Off Widget" gone from PlayStore?

The best (in my opinion) app/widget is no longer on the PlayStore.
What happened?

Here is the exact icon for it:

Anyone know of a clean, reliable, non-pirated site I could get it from?
I had downloaded it from Play many moons ago, and it is not listed in "My Apps" in the PlayStore anymore.

Or, if someone knows of a minimal widget that will do the same thing?
Basically you add the widget to your homescreen, click it, and the screen turns off. Yes I know I can push the power button to turn the screen off, or wait until it times out, but yeah, I'm OCD that way
