Help SD-card isn't recognized by phone/PC. Is it completely dead?

Hi, i have 16gb sdcard in my phone mounted (or formated) as internal storage.

After 4 months, my phone went crazy and i have to re-flash android.

But, when i flashed new android and connected the sdcard to the phone, the card needed to be formated. I tried several programs on pc, tried several phones but nothing can restore my card. I had on that card all my images/files. Some ideas how can i restore it? Is it completely dead?

When you had that card set up as internal (aka adoptable) the file system on it was changed from its default FAT (probably FAT32) to ext4 and it was encrypted. When you re-flashed the ROM on your phone, that wiped the encryption key needed to access your encrypted microSD card. You won't be able to access any of the data on the card at this point.
You can still re-use the card, you just need to reformat it.
