If the SD card fits in the phone, you can do this with a file manager from the Play Store (I use one called File Manager).
Not quite sure why you'd want to move music from and SD card if it fit the phone, so assuming it doesn't:
- put the SD card in a phone that it does fit and connect to a PC
or
use a PC with a card reader
or
get a USB card reader for a PC (available from ebay) and pop the card in that and that in the PC
THEN
- copy the files to the PC
- connect the new phone via a USB cable
- copy the files to the new phone
