Shortcut modification app

ok i have searched and serached. i have a samsung galaxy phone and galaxy tablet. i would like to get a custom app that makes shortcutts to programs, changing both there name and icon. i love the samsung interface and dont need a new desktop. just an icon modifier. anyone out there know of one that works? again i DONT want to modify my main desktop.
