Question, would you be willing to Un Root and S-on this phone?

I'd rather not and here is my reasoning:
Having a fully rooted phone is much more beneficial to the end user.

Including, but not limited to, the ability to make a full backup of the phones current state, changing radios, roms, access to root apps, and access to solutions for troubleshooting over and above what is available being stock.

May I ask why you would want it unrooted/s-on?

Yes, the ESN is clean.:)
As far as unlocked.....the bootloader is unlocked, but not carrier unlocked.
Don't know if you can switch it to TMO or not...I thought TMO was GSM and the Evo is CDMA....I'm not familiar enough with that to tell you it can work or not, sorry ;)
 
If it's a CDMA phone it'll never work on a GSM network.
Basically GSM (Even 4G) have a SIM (Subscriber Identity Module) card which has the phone's number and some security stuff and can store some SMS and has a phonebook for 200 Phone numbers. All that stuff is internal to the phone on CDM (NO provision for a SIM card) so the two technologies are not interchangeable.
