Sound Resets to Vibrate

My sound, without me doing anything that I'm aware of, will reset to vibrate and the ringtone volume will go to 0. I've missed several calls because of this. I have to check all the time to see if it has reset.

What am I doing? What can I do to lock the sound status and ringtone volume?
