Can you maybe take a snap of box and phone and in a mirror (suitably edited to leave yourself out as you prefer) along with the sample info as bobcat331 suggests?

I don't recommend using the front camera to take a picture of itself, bad things happen, lol jk.

I have a HTC evo 3d(Rogers in Canada) just sitting in a drawer. My wife got it when I got mine, then a month later switched back to Blackberry because she missed BBM. Not sure if you can unlock mine or your is unlocked. The EVO is a GSM phone, also has an Otterbox defendor case with it.
