Help Text sent to "No Recipient"

In the messaging app there is a message sent to "No Recipient". How is this possible?

I've tried to replicate it, but the Send button is not accessible until you enter a recipient. You can't save a draft without choosing a recipient. Sending to a recipient and then deleting the contact will still show the phone number or email address in the text message without the contact name.

I thought it could be a hidden contact, but I haven't found a way to "hide" a contact in my list. There wasn't a contact saved as "No Recipient" in the contact list. Could it somehow be a message showing up from a 3rd party messaging app?

Thanks for any insight.

sometimes happens to me when I respond to a group sms that was sent to me by a bunch of co-workers on iphones.
