Time tracking app

I've searched and tested numerous time tracking apps, not finding one I really like.

I'm an IT consultant that charges an hourly rate. Based on the time of day, the hourly rate changes.

I need a simple time tracking app that will allow me to start and stop time. Also go back and modify it. This time needs to be able to be linked with a customer. I don't need all kinds of options for projects, tasks, cars, etc.

I also need to be able to add an item to a customer. This would be used to charge for hardware that I've purchased on their behalf. An area to add a note to a customer would suffice as well.

It would be nice to be able to view and modify online. This isn't needed but would be nice. I especially want a service I can host myself, and not rely on others' servers.

Invoicing is also optional. Don't require it, but if it does it well then +1.

P.S. One other thing, it must not have a monthly fee. Paid up front is fine, but I do not wish to pay monthly for a service.

I consult with about the same requirements. I don't know about a time based rate change though.
Perhaps a "day" project and an "off-hours" project.
I use it everyday and like the layout especially the easy to access "notes" field. This was the best choice based upon my desired "feature list".

It is a phone based app.
