Help unfortunately the process com.android.phone has stopped

I just got my fixed S3 back today (screen crack) and when I went to Bell to get my account transferred back onto it the titled error came up and pops up twice every time I reboot the phone.
The phone was rooted, so I un-rooted it with SuperSU and then did a factory wipe and went through the day one set-up process again.
Still getting the error message.

Any ideas? Again the error is "unfortunately the process com.android.phone has stopped"

I've done searches and tried various things:

deleted contacts and remove all accounts
clear phone cache
clear contacts cache

edit#2:
restored stock firmware (phone had to be rooted, so is now rooted again)
software has just been update OTA

problem/error pop up is still there on reboot.

also, just noticed my data isnt working, just wifi. anyone know the Bell APN info?
 
Ok, I fixed the error, reinstalled stock firmware using Odin3


But my LTE/data is still a no go. First I want to check the APN settings, and if that checks out I'm guessing I need to install a new modem?
Any one on Bell who wouldnt mind posting their APN setting I would really appreciate that!
