Unlock SIM PUK code

I use a Droid Incredible 4G LTE by HTC, and my carrier is Verizon Wireless. I've recently I've accidentally locked myself out by trying to unlock the SIM code. I entered wrong three times and now it is requiring me to enter my eight digit SIM PUK code, and I have no idea what it is. So far when it asked me to do so, I entered a random eight digit number to see what would happen, and it asked me to enter and re-enter a new 4-8 digit SIM lock code. When I did, it told me that the code I've entered doesn't match with the SIM PUK password. Please help!
 
You need the PUK code you got when you bought the SIM for the carrier you had it on. When I got my replacement SIM, the PUK code was on the SIM card card box thing. (The card you got the SIM from lol). I'm looking at my T-mobile SIM card's card and it has the PUK code and SIM number. T-mobile and AT&T should definitely be able to help you or give you a SIM replacement for free though. ;)
